7 injured, 3 of them critically, after fiery blasts rock Florida propane plant

A combination of human error and equipment failure might have caused a series of explosions that engulfed a propane plant in flames in Tavares, Fla., late Monday, injuring eight people and forcing evacuations for a half-mile around the scene, authorities said.

The explosions began as a fire spread through the Blue Rhino plant,?near Orlando,?about 10:30 p.m. ET, Lake County sheriff's Lt. John Herrell told reporters early Tuesday.

Plant officials say all employees of the plant are accounted for. All of those injured were in the plant when the fire started.

Tavares Fire Chief?Richard Keith said Tuesday morning that sabotage is not expected. The fire marshal is on site to do a full investigation.?

Residents of the area told NBC station WESH of Orlando that flames from the explosions could be seen for several miles, but there were no reports of damage to the surrounding buildings. ?

"It sounds like bombs are going off," Norma Haygood, a nearby resident, told WESH.

No deaths were immediately reported, Herrell said. Three male victims were in critical condition, the Orlando Regional Medical Center, told NBC News.?

Two were airlifted; a third was transported by ambulance. ?

Authorities initially declared a one-mile evacuation zone around the plant, but they later scaled that back to a half-mile, effective until 6 a.m. ET.?

"We feel that there is no longer any danger" to the area around the plant, Herrell said.

Twenty-four or 26 people were scheduled to work the night shift at the 33,000-acre facility, according to plant managers, Herrell said. Authorities initially said 15 people were unaccounted for, but Herrell later said the company's management said that they had accounted for all of the people it knew were in the plant at the time of the initial explosion.

Some of them showed up at others location, according to Blue Rhino, Herrell said. He said he had no further information on where the others were.

The plant housed about 53,000 20-pound propane cylinders, Herrell said.

"They store the propane cylinders on different parts of the property, and as the fire spread, there were more and more explosions," he said.?

Explosions continued until 1 a.m. ET Tuesday, roughly 2? hours after the first blasts were reported.

Don Ingram, former plant production supervisor at the plant, said his son felt one of the explosions in their home 6 miles away.

Ingram told WESH that the back area of the plant is "lined with propane tanks stacked four or five high on plastic pallets."

"I don't think you fight this fire," he said. "It's just too dangerous."

Blue Rhino is a subsidiary of Ferrellgas Partners, the second-largest distributor of propane in the U.S.

Angels lose to Texas Rangers, 14-11, on walk-off home run in 10th inning

ARLINGTON, Texas ? It was an extremely eventful Tuesday evening for the Angels, who pounded out 16 hits but lost their fifth consecutive game, blowing a four-run, eighth-inning lead in a 14-11, 10-inning loss to the Texas Rangers at the Ballpark in Arlington.

Manager Mike Scioscia was ejected in the second inning and Howie Kendrick became the ninth player in franchise history to amass 1,000 hits with a two-run single in a four-run eighth inning.

Mike Trout had four hits and two walks and has reached base in 10 consecutive plate appearances, two shy of Bobby Grich's club record of 12, set in 1984, and Josh Hamilton drove in four runs.

C.J. Wilson had his worst start (four innings, six runs, 11 hits) of the year, and the bullpen suffered another meltdown, Dane De La Rosa giving up a three-run home run to Geovany Soto in the eighth inning and struggling closer Ernesto Frieri blowing a save in the ninth for the second consecutive game.

Texas then won it in the 10th inning on Leonys Martin's three-run walk-off home run against Daniel Stange, who walked the first two batters of the inning.

Alberto Callaspo was there for the start, but by the time the 4-hour 46-minute marathon ended, the third baseman was gone, traded to the American League West-leading Oakland Athletics for triple-A infielder Grant Green, a deal that will save the Angels $6.275 million in salary through 2014.

?This guy was one of our favorites, we're going to miss him,? Scioscia said of Callaspo, who is in the first year of a two-year, $8.975-million deal. ?We wish him luck. He'll be a fine addition for those guys.?

Callaspo, a switch-hitter who is batting .252 with five home runs and 36 runs batted in and is a solid defender, was pulled in the fifth inning and missed by the ninth inning.

After Frieri walked Ian Kinsler on four pitches with two outs and Kinsler stole second, Adrian Beltre hit a hard grounder that went under the glove of Tommy Field, who replaced Callaspo at third base.

Kinsler scored to tie the score, 11-11, and the play was ruled a hit that gave Frieri, who has retired only six of the 20 batters he has faced in his last four games, his fourth blown save.

?It's so frustrating the way things are going lately,? Frieri said. ?Everything's been going wrong. Not just for me, for everybody. This is the most frustrating moment of my career.?

Not for Callaspo, who gained 14 games in the standings with Tuesday night's trade. Callaspo, 30, is expected to share a second-base platoon with Eric Sogard in Oakland, though he hasn't played second regularly since 2009 with Kansas City.

?We need some help from the right side swinging,? Oakland General Manager Billy Beane told A's reporters. ?It's become noticeable in the last month or so.?

Green, 25, will report to triple-A Salt Lake. The former Anaheim Canyon High and USC star, a first-round pick (13th overall) in 2009, was hitless in 15 at-bats in a recent big league stint.

Green hit .325 with 11 home runs and 50 RBIs in 87 games for triple-A Sacramento this season but has struggled to find a position. He was drafted as a shortstop but played the outfield, third base, first base and second base in the minor leagues.

?He'll get some time at second, third and shortstop, and we'll see where it goes,? said Matt Klentak, Angels assistant general manager. ?We believe his bat profiles to being a big league regular, for sure. He's hit at every level he's played at.?

The Angels pulled triple-A infielder Chris Nelson from Salt Lake?s game against Iowa on Tuesday. He is expected to take Callaspo?s place on the Angels roster.

Pegatron and Apple (AAPL) issued statements on Monday responding to a report from China Labor Watch alleging widespread workers' rights violations at three Pegatron facilities.?

Apple surveys working hours for more than 1 million employees across our supply chain each month and we report the findings on our website.

We have been in close contact with China Labor Watch for several months, investigating issues they've raised and sharing our findings. When they first told us that workers' ID cards were being withheld, an auditor from our Supplier Responsibility program was on-site the next day to investigate. We confirmed that labor brokers for Pegatron were holding a small number of IDs as they helped set up bank accounts for those employees. We demanded Pegatron put a stop to this practice and a new system was in place within a week.

Their latest report contains claims that are new to us and we will investigate them immediately. Our audit teams will return to Pegatron, RiTeng and AVY for special inspections this week. If our audits find that workers have been underpaid or denied compensation for any time they've worked, we will require that Pegatron reimburse them in full.

Our audits involve a thorough review of timecards and other documents to guard against falsification. We will investigate these new claims thoroughly, ensure that corrective actions are taken where needed and report any violations of our code of conduct. We will not tolerate deviations from our code.

College Football Players Killed In Crash

Texas A&M said Tuesday that redshirt freshman Polo Manukainiu and a friend who was joining the Utah football team this fall were among three people killed in a single-car rollover crash in the high desert of northern New Mexico, stunning both schools just days before fall practices begin.

Manukainiu, a 19-year-old defensive lineman for the Aggies, and 18-year-old Utah recruit Gaius "Keio" Vaenuku were killed, officials from both schools said.

The wreck happened Monday evening on U.S. 550 near Cuba, about 85 miles north of Albuquerque, apparently as the group was returning to suburban Dallas, where three of them had ties to prep football power Trinity High School in Euless.

New Mexico state police said Manukainiu and Vaenuku were passengers in a southbound 2002 Toyota Sequoia that drifted off the sagebrush-lined highway, according to Texas A&M. The driver overcorrected and the vehicle rolled several times.

Alcohol wasn't involved and it appeared that the driver, who was not immediately identified, was the only one wearing a seatbelt, authorities said.

Manukainiu and passenger Andrew "Lolo" Uhatafe died at the scene after they were ejected from the vehicle, the Texas A&M statement said. Vaenuku was pronounced dead in an ambulance that responded to the accident.

Survivors included Salesi Uhatafe and his son Salesi Uhatafe Jr., an incoming freshman football player for Utah and a stepbrother of Manukainiu. Both suffered only minor injuries, authorities said.

Manukainiu had apparently traveled to Salt Lake City for some relaxation, tweeting Sunday: "It's always good to get away from the Texas Heat for the weekend. Utah got that breezeeeeeee."

On Monday, hours before the accident, he tweeted: "22 hour drive back to Texas on no sleep. Oh my."

Manukainiu played football at Trinity High School in Euless, west of Dallas, and was part of the Aggies' 2012 signing class. He was a recreation, parks and tourism science major, the school said, and is survived by his mother, Lima Uhatafe of Euless.

"We lost a terrific young man," Texas A&M coach Kevin Sumlin said. "Polo was loved by his teammates and coaches. Anyone who came in contact with him was struck by his sense of humor and smile. My heart aches for his mom and family members."

Texas A&M finished last season ranked No. 5 after an 11-2 season, their first in the Southeastern Conference. They were led by quarterback Johnny Manziel, who became the first freshman to win the Heisman Trophy, and are expected to be highly ranked again this year.

"Heart hurts waking up to news about Polo," Manziel tweeted on Tuesday. "I think I speak for everyone on our team when I say we love you brother you will be missed."

It is the second such tragedy for Texas A&M in less than two years: Senior offensive lineman Joseph Villavisencio, 22, was killed in a December 2011 car accident after veering head-on into the path of an 18-wheeler 40 miles from College Station. He had spent part of that day delivering gifts to families at a local shelter. Manziel mentioned Villavisencio during his Heisman acceptance speech last year.

Vaenuku was a defensive tackle who had planned to play one year at Utah before going on a two-year Mormon mission, the Deseret News of Salt Lake City reported in January when he committed.

"Everyone who knew Gaius is heartbroken today," Utah coach Kyle Whittingham said. "He was the kind of young man who lit up a room and his future in football and life had no boundaries. Words cannot express our devastation over the loss of Gaius."

Vaenuku was a member of The Church of Jesus Christ of Latter-day Saints and considered playing at church-owned Brigham Young but he said he felt more at home in Salt Lake City playing for the Utes. His bio on the Utah website says Vaenuku he had three brothers and three sisters and enjoyed acting and singing.

The news stunned Trinity High in Euless, where Manukainiu, Vaenuku and Uhatafe Jr. all played football. The team has been one of the best in Texas in recent years, with three state titles in a span of five seasons from 2005-09 and a trip to the championship game in 2010.

Principal Mike Harris said the deaths have affected a majority of the Euless community, where there is a tight-knit Polynesian community.

"They were students with bright smiles that everybody knew and everybody loved," the principal said.

Texas A&M associate athletic director Alan Cannon said Manukainiu was known for his sense of humor and "will be sorely missed as a person you enjoyed being around." He said the football staff was working Tuesday to notify teammates of his death. Players are scheduled to report to campus Sunday to begin practicing for the upcoming season.

Cannon said Manukainiu was to wear jersey No. 90 and that it was too early to determine if players will affix the number to their uniforms as a tribute. The NCAA must approve any such recognition, Cannon said.

Swimming: China's Sun adds world 400m gold to Olympic crown

BARCELONA: China's Sun Yang proved he has few peers in the distance freestyle events on Sunday, while US star Missy Franklin claimed the first of what could be eight golds at swimming's world championships.

The 21-year-old Sun won the world men's 400m freestyle gold to complete the set of major titles in the event, 12 months after winning the Olympic crown.

Just as he did in the London 2012 final, the 21-year-old Chinese dominated early on in Barcelona to add the 400m world title to the 800m and 1500m freestyle golds he won two years ago in Shanghai

"The 400m is a very important event and I completed my goal," said Sun, who has moved to Hong Kong in the wake of a fallout he had concerning his girlfriend, an air stewardess, with his previous coach.

"I overcame many problems and proved myself, I hope to have an even better performance in my other events," he said as he prepares to start the defence of his 800m freestyle title in Tuesday's heats.

Sun clocked 3min 41.59sec, finishing more than three seconds ahead of the field, to cap a bright start on the first of the eight-day long championships for China as Ye Shiwen eased into Monday's women's 200m individual medley final.

The 17-year-old Ye, who won 200 and 400m IM gold Olympic medals last year, sparked controversy when she swam the final leg of her 400m triumph in a faster time than men's winner Ryan Lochte at London 2012.

Defending champion Ye was the second quickest qualifier, just over half a second behind Hungary's Katinka Hosszu, the fastest out of Sunday's 200m IM semi-finals.

The USA claimed two world titles on Sunday as 16-year-old Katie Ledecky just missed the women's 400m freestyle world record on her way to winning gold, finishing more than two and a half seconds ahead of her rivals.

The teenager, who enjoyed a shock win in the women's 800m freestyle at the Olympics, just missed out on breaking the four-year-old world record.

"I really wasn't expecting to go that fast, that wasn't my focus. I just wanted to compete with these girls and get the gold medal," said Ledecky, who will also race in the 800 and 1500m.

The Americans kept on a role as Franklin swam the first leg to lead the USA to the women's 4x100m freestyle gold.

Franklin, who won four golds at London 2012, is bidding to become the first swimmer -- male or female -- to win eight gold medals at a world championship, a feat not even 18-time Olympic gold medallist Michael Phelps achieved.

The US teenager will swim in the 50, 100 and 200m backstroke events, the 100 and 200m freestyle, with two more relays to come.

The USA won on Sunday by just 0.12 of a second from the Australians.

It was Megan Romano, swimming the anchor leg, who managed to hold off Alicia Coutts, leaving the Australian in floods of tears.

The USA nearly made it a hat-trick of titles in the men's 4x100 freestyle final, but France's Jeremy Stravius hit the wall just 0.24 seconds faster than America's James Feigen as the Olympic champions added world gold.

In a grandstand finish, four teams were still in contention, but the French hung on amidst plenty of vocal support from travelling fans.

Sweden's Sarah Sjostrom was the fastest into Monday's final of the women's 100m butterfly as the 2009 world champion clocked 57.10sec ahead of title holder, Olympic champion and world-record holder Dana Vollmer of the USA.

Vollmer was fourth fastest in the semi-finals, with Australia's Olympic bronze-medallist Coutts third.

South Africa's Olympic 100m breaststroke champion Cameron van der Burgh was joint-second fastest into Monday's final behind Australia's Christian Sprenger, the London silver medallist, who clocked 59.23 sec in the semi-finals.

    Arizona firefighters young, dedicated, beloved

    The 19 firefighters killed Sunday were young, brave family men.

    Many followed in family footsteps when they decided to dedicate their lives to battling deadly blazes. And a number of those lost were in the beginning stages of starting their own families before attempting to calm a fast moving fire in central Arizona on Sunday.

    They were a group known for being tough as nails?but with a lighter side that was needed to cope with the demands of the job.

    But now, many leave behind wives and children who now understand all too well the sacrifices their loved ones made when they became firefighters. Here are their stories.

    Anthony Rose, 23
    In just a few months, Anthony Rose would have been a father.

    His?fianc?e,?Tiffany Hettrick, was due in October, family members told NBC News. The couple lived in Arizona, and Hettrick had been accustomed to her husband-to-be's being gone doing what he loved.

    ?He always wanted to be a firefighter, and he was living what he wanted to do. He?s with God now, so he?s OK,? Hettrick's stepfather, Michael Mooney, told NBC News.??I don?t warm up to too many people too much, but he was a very good guy, and I knew that right away.??

    Sean Misner, 26
    Like many firefighters, Sean Misner had the job in his blood.?

    The California native's uncle and grandfather were firefighters around Santa Barbara, Calif., according to the Santa Barbara Independent.?

    Misner had been married less than one year, and the couple were expecting their first child soon, the paper reports.

    ?The devastating loss of nineteen lives in the Arizona wildfire on June 30, 2013, has not only affected the fire service overall, but one of our own Montecito Fire families,? the Montecito Fire Protection District announced Monday. ?We send our heartfelt condolences to all the families who lost loved ones in this tragedy."

    Christopher MacKenzie, 30, had always wanted to follow in his father's footsteps and become a firefighter.

    The son of a firefighter, Christopher MacKenzie lived life to the fullest, friends say. He was an avid snowboarder who always wanted to follow in his father's footsteps.

    He grew up in California's San Jacinto Valley and became a member of the town's fire department. He hurt his leg during the first week on the job and ended up in the hospital, only emerging more determined than ever to spend his life battling intense blazes, his mother said.

    "I was very proud of him. Yeah, he was a good kid, and he's gonna be so missed," said Laurie Goralski, MacKenzie's mother.?

    Kevin Woyjeck, 21, hoped to one day join the Los Angeles County Fire Department.

    Kevin Woyjeck's goal was to one day join the Los Angeles County Fire Department, just like his dad, Joe Woyjeck.

    "We're a giant family. Any firefighter lost throughout the country is one of us. We're all a big family," Keith Mora, L.A. County fire inspector, told reporters.

    The Southern California man began his short career as a member of the Fire Explorers, a mentorship program for young men and women who wanted to become firefighters, according to NBC Los Angeles.

    "He was doing everything he could to become a professional firefighter -- he had an extreme work ethic," Mora told NBC Los Angeles. "He was a great, great kid. I say kid, but he was a young man at 21 years old."

    Andrew Ashcraft, 29
    Andrew Ashcraft leaves behind a wife and four children, none of whom is older than 6.

    Friend Jeannie Brown remembered him as a "good husband, good, devoted family man." She knew Ashcraft through her husband and said the firefighter was always quick to help.

    His wife, Juliann Ashcraft, told TODAY on Tuesday that her husband had a ?contagious smile, a heart of gold.?

    ?He wanted to protect the community that he lived in and loved,? she said. ?In our time of mourning we want people to know that they were heroes, that they are heroes.?

    Dustin Deford, 24
    Dustin Deford is remembered as a work horse who would do anything to help a neighbor.

    ?This whole community knows him, not a lazy bone in his body,? Sheriff?Neil Kittleman, who was Deford's boss at a hazardous fuel reduction program in Montana from 2007 to 2012, told NBC News.

    ?He?d help you do anything,? said?Kittleman.

    Deford, who was not married, comes from a large family, according to Kittleman. He father is?pastor at a church in Ekalaka, Mont.

    Eric Marsh, 43
    Eric Marsh had been involved with firefighting since graduating from high school in Ashe County, N.C., according to the Jefferson Post.

    Marsh went on to graduate from Appalachian State University, where he studied biology.

    "He was a great son. He was compassionate and caring about his crew," John Marsh, Eric's father, told the newspaper.

    Eric Marsh was married but had no children.

    William Warneke, 25
    Like some of his Hotshot colleagues, Marine Corps veteran William Warneke was preparing to become a father, his grandmother Nancy Warneke told The Press-Enterprise newspaper in Riverside, Calif.

    Warneke and his wife, Roxanne, were expecting their first child in December. The?Hemet, Calif., native joined the Hotshot crew in April after four years in the military that included a tour in Iraq.

    The couple had just purchased a property in Prescott near family.

    Nancy Warneke told the newspaper that she called William's sister after seeing reports of the fire on the news.

    ?She said, ?He?s gone. They?re all gone,?? she said. ?Even though it?s a tragedy for the whole family, he was doing what he loved to do. He loved nature and was helping preserve nature.?

    Scott Norris, 28
    Scott Norris seemed to always have a smile on his face, according to friends.

    "He talked to random people and had a conversation with them," said 19-year-old Ryan O'Hara. "That's the type of guy he was, and honestly, it inspired me, in a way, to be like that."

    Throughout Prescott, he was best known for his part-time job at Bucky O'Neill Guns.

    "Here in Arizona the gun shops are a lot like barber shops. Sometimes you don't go in there to buy anything at all, you just go to talk," resident William O'Hara told the Associated Press. "I never heard a dirty word out of the guy. He was the kind of guy who if he dated your daughter, you'd be OK with it."

    Travis Carter, 31
    Travis Carter was a fitness buff whose strength was legendary at a local gym.

    At Captain Crossfit gym, Carter gained a reputation for being the strongest of the Hotshots and also the nicest, friends told the Associated Press.

    ?No one could beat him. But the thing about him, was he would never brag about it. He would just kill everyone and then go and start helping someone else finish,??trainer Janine Pereira told the AP.

    He made up demanding workouts, motivated fellow Hotshots to push themselves and gained?notoriety?for holding a plank (an exercise in which a person gets in a push-up position and holds himself up on toes and forearms) for 45 minutes, the AP reported.?

    John Percin, 24
    John Percin was a high school athlete known for his laugh and penchant for fun, friends told The Oregonian newspaper.

    The 24-year-old was a 2007 graduate of West Linn High School, about 20 miles south of Portland, Ore., the paper reports. He grew up playing baseball.

    "It's a tough day for us today," Percin's father, John Percin Sr., told The Oregonian. "We don't know a lot of things yet."

    Robert Caldwell, 23
    When it came to fighting flames, Robert Caldwell was a natural, friends say. He not only possessed the physical?stamina?for the grueling work but also had the smarts.

    "He was one of the smart guys in the crew who could get the weather, figure out the mathematics. It was just natural for him," Chase Madrid, who worked as a Hotshot in the two prior years, told the Associated Press.

    Caldwell's understanding of the science behind raging fires helped him get appointed as a squad boss.

    Travis Turbyfill, 27
    Known as "Turby" to his squad, Travis Turbyfill got his job with the Hotshots when another member's girlfriend asked him to quit.

    Like many of the 18 other firefighters who perished Sunday, Turbyfill frequented the Captain Crossfit gym near the Hotshot headquarters. ?Trainer Tony Burris said the firefighter would often come in with his two daughters.

    "Because he's this big, huge Marine, Hotshot guy, and he has two little girls, reddish, blonde curly hair, and they just loved their dad," he told the Associated Press.

    Wade Parker, 22
    Wade Parker had always wanted to be a second-generation firefighter, according to family friends.?Parker's father works for the nearby Chino Valley Fire Department, family friend Jeff Knotek told the Associated Press.

    Knotek called Parker, a "big, athletic kid who loved it, aggressive, assertive and in great shape."

    "It's just a shame to see this happen," he said.

    Grant McKee, 21
    A giving young man who whose mother told the AP he was in training to be an emergency medical technician, McKee would regularly request extra emergency room shifts.

    ?I had four grandchildren, but Grant was the sweetest most giving nature of any of my grandkids,? his grandmother Mary Hoffman told the AP. ?We used to think he was a little angel.??

    McKee?s cousin, Robert Caldwell, was also among those killed on Sunday.

    Jesse Steed, 36
    Among the older members of the Hotshots crew, Steed was remembered for his ability to always make his coworkers laugh ? but he couldn?t have been more serious about doing his job right.

    Steed?s brother Cassidy Steed spoke of his brother?s commitment to helping other in a statement released on Monday, according to Seattle?s KOMO News.

    ?Jesse has always put his life on the line for people who he knew he would never meet. He sacrificed time with his family and his own personal interests. Jesse loved his job very much as his family supported him with every sacrifice he made for it,? Cassidy Steed said in the statement.

    ?All of Jesse?s crew also gave the ultimate sacrifice. Now there are 19 families who are as grief-stricken and numb from their recent loss as I am,? he said.

    Clayton Whitted, 28
    A high school football player whose coach recalled him as a ?wonderful kid,? Whitted never shirked a challenge.

    ?He was a smart young man with a great personality, just a wonderful personality,? remembered Lou Beneitone, who coached Whitted at Prescott High School. ?When he walked into a room, he could really light it up.?

    Garret Zuppiger, 27
    Zuppiger was ?always, always willing to lend a helping hand,? former neighbor Frank Morrison told The Arizona Republic. ?He would give you the shirt of his back.?

    The man who wrote ?Everyday is like a gift!? on a blog he maintained also had a passion for a good beer and the ginger-hued facial hair he cultivated, according to the AP.

    ?We both had a red beard and so we would always admire each other?s beards,? friend Tony Burris told the AP.
